Licensed Practical Nurse (LPN)

Assist with health assessments, participant wellness monitoring, and medication administration while maintaining accurate documentation and physician orders. Support the development of individualized care plans and promote health education and preventative care practices. Collaborate with registered nurses, physicians, specialists, families, and interdisciplinary team members to ensure coordinated care and contribute to positive participant outcomes in a safe, person-centered environment. Participate in care planning meetings to drive positive outcomes for adults and seniors with complex medical or rehabilitative needs.
