Job Summary
Conduct direct nursing care for residents with complex behavioral needs, including administering medications, performing procedures like catheterization and dressing changes, and monitoring vital signs. Supervise nursing assistants and certified nursing assistants, ensuring all staff comply with facility policies and safety regulations. Manage administrative duties such as charting, completing incident reports, maintaining census records, and coordinating with physicians for treatment plans. Perform daily rounds to evaluate resident status, verify care plan adherence, and address complaints or grievances. Admit, transfer, and discharge residents as required while maintaining strict confidentiality and infection control protocols.
Required Qualifications
- Licensed Practical Nurse (LPN)
- Ohio Nurse Practice Act scope of practice
- Personal protective equipment use for blood/body fluid exposure
- Knowledge of federal, state, and local standards governing long-term care facilities
- Ability to direct day-to-day functions of nursing assistants
- Ability to ensure compliance with written policies and procedures
- Ability to review and recommend revisions to department policies and procedure manuals
- Ability to plan shifts' services, programs, and activities
- Ability to interpret department policies and procedures for personnel, residents, visitors, and government agencies
- Ability to admit, transfer, and discharge residents
- Ability to complete accident/incident reports
- Ability to write resident charge slips
- Ability to maintain and submit Daily Census Report
- Ability to complete medical forms, reports, evaluations, studies, and charting
- Ability to complete and file required recordkeeping forms/charts upon admission, transfer, and/or discharge
- Ability to receive and record telephone orders from physicians
- Ability to transcribe physician's orders to resident charts, cardex, and medication cards
- Ability to chart nurses' notes in an informative and descriptive manner
- Ability to fill out and complete transfer forms
- Ability to ensure appropriate documentation concerning unauthorized discharges
- Ability to prepare and administer medications as ordered by a physician
- Ability to verify resident identity before administering medication
- Ability to ensure prescribed medication for one resident is not administered to another
- Ability to ensure adequate supply of floor stock medications, supplies, and equipment
- Ability to order prescribed medications, supplies, and equipment
- Ability to ensure narcotic records are accurate
- Ability to notify the Nurse Supervisor of drug and narcotic discrepancies
- Ability to review medication cards for completeness and accuracy
- Ability to notify the attending physician of automatic stop orders
- Ability to dispose of drugs and narcotics as required
- Ability to participate in employee performance evaluations
- Ability to determine shift staffing requirements
- Ability to make recommendations concerning employee dismissals and transfers
- Ability to inform the Nurse Supervisor of staffing needs
- Ability to report absentee call-ins
- Ability to develop work assignments
- Ability to provide leadership to nursing personnel
- Ability to make daily rounds of the unit/shift
- Ability to ensure nursing service personnel are performing work assignments in accordance with acceptable nursing standards
- Ability to ensure certified nursing assistants are under direct supervision of a licensed nurse
- Ability to meet with shift nursing personnel to identify and correct problem areas
- Ability to ensure departmental personnel, residents, and visitors follow established policies and procedures
- Ability to create and maintain an atmosphere of warmth, personal interest, and positive emphasis
- Ability to review complaints and grievances
- Ability to receive/give the nursing report upon reporting in
- Ability to report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals
- Ability to inform nursing personnel of new admissions and room assignments
- Ability to participate in the orientation of new residents/family members
- Ability to make rounds with physicians
- Ability to requisition and arrange for diagnostic and therapeutic services
- Ability to consult with the resident's physician
- Ability to review the resident's chart for specific treatments, medication orders, and diets
- Ability to implement and maintain established nursing objectives and standards
- Ability to make periodic checks to ensure certified nursing assistants are properly administering prescribed treatments
- Ability to evaluate the resident's physical and emotional status
- Ability to cooperate with social and activity programs
- Ability to notify the resident's attending physician and next-of-kin when there is a change in condition
- Ability to carry out restorative and rehabilitative programs
- Ability to inspect nursing service treatment areas daily
- Ability to administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages, and range of motion exercises
- Ability to use restraints when necessary
- Ability to obtain sputum, urine, and other specimens for lab tests
- Ability to take and record TPRs and blood pressures
- Ability to monitor seriously ill residents
- Ability to check foods brought into the facility by family/visitors
- Ability to report problem areas to the Nurse Supervisor and Dietary Supervisor
