Licensed Practical Nurse (LPN) at Sandy Ridge Center for Rehabilitation and Healing for full-time night shift (11PM-7AM). Responsibilities include admissions, discharges, and transfers of residents; medication requisitions with pharmacy and physicians; medication administration; adherence to infection control and center policies; collaborating with the interdisciplinary team to achieve optimal resident outcomes; identifying and reporting abnormal changes in residents’ condition per center policy; working under direct supervision per state Nurse Practice Act and facility policies; delivering nursing care for residents requiring long-term or rehabilitative care; collecting patient data, making observations, and documenting care; implementing and evaluating the patient/resident plan of care per state regulations; directing and supervising other nursing personnel as appropriate; maintaining necessary documentation and knowledge of equipment.”,
