Job Summary
Collect and record resident health information, including vital signs and medical details. Administer medications and injections as prescribed by providers. Provide wound care, assist with dressing changes, and support residents with personal care and daily living activities. Monitor and document nutritional and fluid intake/output. Supervise Certified Nursing Assistants (CNAs) as needed and collaborate with the nursing team to maintain care standards. Valid New York State LPN license required; prior long-term care experience preferred.
