Job Summary
As an LPN at Resthaven, provide direct nursing care and implement individualized resident care plans; administer medications, treatments, and pain management interventions; monitor resident health status including vital signs, wound care, and skin integrity; observe and report changes in condition; document accurately in the electronic health record and ensure infection control and regulatory compliance; respond to urgent situations with clinical judgment; communicate professionally with residents, families, physicians, and the care team; lead and support CNA and clinical staff, assist with staffing coverage, and deliver comprehensive shift reports; float between units based on operational needs while delivering resident-centered service.
Required Qualifications
- High school diploma or GED required
- Graduate of an accredited Licensed Practical Nurse (LPN) nursing program
- Successful completion of State Board of Nursing licensure examination
- Current, active Michigan Board of Nursing License as a Licensed Practical Nurse (LPN)
- Previous LPN/RN nursing experience preferred, ideally in long-term care, skilled nursing, rehabilitation, assisted living, or senior care.
- Experience working with geriatric or elderly populations preferred
- Must maintain all continuing education requirements to keep nursing licensure active and in good standing
- Participates in ongoing learning, professional development, and required in-service training
- Proficient with electronic documentation systems and healthcare technology
