Job Summary

LPN Rehab Nurse working full-time on a 12-hour night shift in a long-term care/rehab setting. Provides basic nursing care, conducts resident assessments on admission and ongoing basis, updates and communicates the care plan to the IDT and residents/families, administers medications and treatments safely and timely (IV administration if IV certified), documents interventions accurately, uses SBARR for timely physician notification, maintains composure during crises, monitors care plan delivery, identifies and addresses problem areas through QAPI, supports ADLs and resident education, and contributes to ongoing nursing education. May be assigned other duties as appropriate.

Required Qualifications

  • Education from an accredited school of nursing resulting in becoming a Licensed Practical Nurse
  • Current license in the state you will working as a Licensed Practical Nurse
  • IV Certification required or willing to obtain within six months of employment
  • Current CPR/AED Certification required or willing to obtain within six months of employment
  • Must be able to assess the basic nursing care needs of the geriatric resident
  • Excellent communication skills to interact with residents, visitors and staff
  • Critical thinking skills
  • Must possess time management and organizational skills
