Job Summary
Assess resident conditions on admission and ongoing basis, communicating significant changes to the interdisciplinary team. Accurately document assessments and participate in care planning, updating plans when interventions require revision. Implement nursing orders, physician directives, and educational needs while administering IV fluids if certified. Monitor residents closely to identify behavioral changes, answer call lights, and assist with activities of daily living. Use critical thinking to address problem areas via the QAPI process and notify physicians using SBARR during crises. Maintain composure in stressful situations and complete twelve hours of continuing education annually.
Required Qualifications
- Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse
- Current license in the state you will be working as a Licensed Practical Nurse
- IV Certification required or willing to obtain within six months of employment
- Current CPR/AED Certification required or willing to obtain within six months of employment
- Must be able to assess the basic nursing care needs of the geriatric resident
- Must possess excellent communication skills to interact with residents, visitors and staff
- Excellent time management and organizational skills are required
- Must possess critical thinking skills
Desired Qualifications
- Long-term care/Rehab/Assisted Living experience preferred
