Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults. Perform ongoing patient assessments, administer prescribed medications and therapies, and coordinate care with families and providers. Educate patients and family members to support improved outcomes while responding appropriately to changes in patient condition. Ensure patient safety through preventive measures and deliver care in accordance with physician orders and plan of care.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (South Carolina)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
