Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults, including ongoing patient assessments, administering prescribed medications and therapies, and delivering care per physician orders. Coordinate with families and providers to educate patients on improved outcomes and implement preventive safety measures. Respond appropriately to changes in patient condition while maintaining one-on-one, patient-centered care. Requires an active, unrestricted RN or LPN license in South Carolina acquired within the past year. Full-time, part-time, and PRN shifts available with 8, 10, 12, or 16-hour options.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (South Carolina)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
