Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults. Perform ongoing patient assessments, administer prescribed medications and therapies, and deliver care according to physician orders. Coordinate with families and providers while educating them to support improved outcomes and implement preventive safety measures. Respond appropriately to changes in patient condition. Full-time, part-time, and PRN opportunities with flexible scheduling including 8, 10, 12, or 16-hour shifts. Pay range $30-$35/hour with referral bonus eligibility.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (Pennsylvania)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
