Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults. Perform ongoing patient assessments, administer prescribed medications and therapies, and deliver care according to physician orders. Coordinate with families and providers while educating patients to support improved outcomes. Respond appropriately to changes in patient condition and implement preventive safety measures. Maintain active, unrestricted RN or LPN licensure in Pennsylvania with at least one year of experience. Full-time, part-time, and PRN shifts available with flexible scheduling including days, nights, and weekends.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (Pennsylvania)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
