Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults, including ongoing patient assessments, administering prescribed medications and therapies, and delivering care per physician orders. Coordinate with families and providers while educating patients to support improved outcomes and implementing preventive safety measures. Respond appropriately to changes in patient condition and maintain meaningful relationships with patients and families. Full-time, part-time, and PRN shifts available with flexible scheduling, including days, nights, and weekends.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (Pennsylvania)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
