Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults, performing ongoing patient assessments and administering prescribed medications and therapies. Deliver care in accordance with physician orders while coordinating with families, providers, and care teams. Educate patients and family members to support improved outcomes and implement preventive measures to ensure patient safety. Respond appropriately to changes in patient condition. Full-time, part-time, and PRN opportunities with flexible scheduling including 8, 10, 12, or 16-hour shifts.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (South Carolina)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
