Licensed Practical Nurse (LPN) / Registered Nurse (RN) – Private Duty Pediatric Nurse

Provide skilled nursing care in a home health setting for medically fragile children and young adults, including performing ongoing patient assessments, administering prescribed medications and therapies, and delivering care in accordance with physician orders. Coordinate care with families and providers while educating patients and family members to support improved outcomes and implementing preventive measures to ensure patient safety. Respond appropriately to changes in patient condition and maintain one-on-one, patient-centered care in the home setting.
