Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults, performing ongoing patient assessments and administering prescribed medications and therapies. Deliver care in accordance with physician orders while coordinating with families, providers, and care teams to ensure patient safety and quality outcomes. Educate patients and family members to support improved recovery and respond appropriately to changes in patient condition. This role offers flexible full-time, part-time, or PRN scheduling with 8, 10, 12, or 16-hour shifts available, including days, nights, and weekends.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (Tennessee)
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
