Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults, performing ongoing patient assessments and administering prescribed medications. Coordinate care with families and providers while educating patients to support improved outcomes and implementing preventive measures for safety. Respond appropriately to changes in patient condition in accordance with physician orders. Full-time, part-time, and PRN shifts are available in Beaver Falls, PA, with flexible scheduling including days, nights, and weekends.
Required Qualifications
- Active, unrestricted Licensed Practical Nurse (LPN) license (Pennsylvania)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
- Strong communication and clinical decision-making skills
Desired Qualifications
- Pediatric or home health experience
- Experience with high-acuity skills (trach, vent, G-tube)
