Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while assessing and documenting conditions such as vitals, glucose, and wound status. Administer medications, insulin, and IV fluids, and perform hygiene assistance and dressing changes. Collaborate with providers and allied health professionals to monitor recovery and report changes as needed. This full-time, part-time, or PRN role in Fombell, PA, requires a current LPN license, CPR/AED/BLS certification, and reliable transportation. No overtime is required. Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as a LPN/LVN in the state position is located
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
