Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while assessing and documenting conditions such as vitals, wounds, and glucose levels. Administer medications, insulin, and IV fluids, and perform wound care and personal hygiene assistance. Monitor patient progress, report changes to providers, and collaborate with allied health professionals to ensure recovery. Work M-F daylight hours in Fombell PA with flexible full-time, part-time, or PRN scheduling. Requires current LPN license, CPR/AED/BLS certification, and reliable transportation.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as a LPN/LVN in the state position is located
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
Desired Qualifications
- Practical trach and/or ventilator experience
