Licensed Practical Nurse (LPN) - Private Duty Home Care

Provide in-home direct patient care according to the provider's prescribed plan of care while monitoring progress and recovery. Assess and document patient conditions including vitals, blood work, and glucose levels, and administer medications, insulin, and IV fluids. Inspect patients for injuries or bedsores, change dressings, and assist with personal hygiene. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. This role offers M-F daylight shifts in Fombell, PA with flexible full-time, part-time, or PRN scheduling.
