Job Summary
Provide direct and indirect patient care under the direction of RNs, NPs, PAs, or physicians, functioning as part of the interdisciplinary team. Collect and record relevant health data, evaluate problems, and implement documented interventions while adhering to research-based standards. Perform Point of Care testing, maintain reagents and equipment, and advocate for patient autonomy and dignity. Ensure informed consent is signed for procedures and practice within New York State-defined scope. Maintain current licensure, BLS certification, and complete required annual training and credentialing. May make home visits and perform other duties as requested. Occasional travel required.
Required Qualifications
- Graduate of a Practical Nursing program
- Current NYS LPN License and registration as a Practical Nurse or a valid interim permit
- BLS certification
- Practices within the New York State-defined LPN scope of practice 100% of the time
- Maintains current licensure, certification, and competencies
- Provides proof of current BLS validation to supervisor
- Completes required unit credentialing according to departmental policy
- Provides to supervisor evidence of completion all required annual training and competency validation
- Heavy Work - Exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs. of force frequently, and/or up to 20 lbs. of force constantly to move objects
- Occasional climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping
- Visual acuity (color, depth, perception)
