Job Summary
Administer and monitor medications as prescribed, conduct basic health assessments including vital signs, and observe client responses to report concerns. Collaborate with therapists, case managers, and providers to support treatment plans and coordinate with pharmacies and external services. Maintain accurate documentation in the EHR, complete medication logs, and ensure compliance with Medicaid and organizational standards. Provide education to clients and families on medication and wellness, and participate in team meetings and care coordination discussions. Follow direction from supervising RN or clinical leadership.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of Georgia
- Knowledge of medication administration and monitoring
- Strong documentation and organizational skills
- Ability to work independently and as part of a multidisciplinary team
Desired Qualifications
- Experience working with behavioral health populations preferred
- Experience in community-based behavioral health or CORE services
- Familiarity with Medicaid documentation and compliance requirements
- Experience with EHR systems (e.g., Credible EHR)
- Bilingual or experience working with diverse populations
