Job Summary

Provide skilled professional nursing services to pediatric and young adult patients in their home setting. Assess patients according to their plan of care, follow physician orders, and manage care for seriously ill, long-term, disabled, and terminally ill individuals. Initiate preventative and rehabilitative procedures while educating patients and families on meeting their needs and goals. Document all care interventions and coordinate interdisciplinary efforts in compliance with state, HIPAA, and agency standards. Notify supervisors of significant condition changes and participate in quality improvement, utilization review, and staff orientation activities.

Required Qualifications

  • Licensed Practical Nurse (LPN)
  • Thorough understanding of state & HIPAA regulations
  • Ability to assess patients according to their plan of care
  • Ability to follow plan of care according to physician orders
  • Experience managing care of seriously ill, long-term, disabled, and terminally ill patients
  • Ability to evaluate patient's treatment/care plans
  • Ability to coordinate interdisciplinary care
  • Ability to obtain required physician orders from a clinical supervisor
  • Ability to provide ongoing assessment of the patient and patient's equipment needs
  • Ability to initiate preventative and rehabilitative nursing procedures
  • Ability to educate and instruct the patient and patient's family
  • Ability to counsel patient and family in meeting patient's needs and goals
  • Ability to notify the clinical supervisor and family of significant changes in the patient's condition
  • Ability to prepare timely, legible, and complete documentation of all care, interventions, and care coordination
  • Awareness of and sensitivity to patient/family rights
  • Ability to apply safety principles as identified by established policy
  • Ability to participate in quality improvement and utilization review activities
  • Ability to participate in the orientation and training of new staff
  • Ability to participate in continuing education
  • Ability to stay abreast of current technologies/practices
  • Ability to participate in establishing priorities, and implementing and evaluating quality improvement efforts utilizing Quality Assurance & Performance Improvement (QAPI)
  • High degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals
