Licensed Practical Nurse (LPN) - Part-Time 3P-11P

Admissions, discharge, and transfers of residents; medication administration and requisitions with pharmacy and physicians; delivering nursing care to patients requiring long-term or rehabilitative care. Collects patient data, makes observations, and reports pertinent information related to resident care. Identifies and reports abnormal changes in residents' condition to proper personnel per center policy. Collaborates with the interdisciplinary team to provide best outcomes while following infection control, emergency, and center policies. Implements the patient/resident plan of care according to state-specific regulations and directs care given by other nursing personnel in selected situations. Maintains knowledge of documentation requirements, equipment maintenance, and confidentiality of resident information. Works under direct supervision in accordance with the state-specific Nurse Practice Act and facility policies.
