Licensed Practical Nurse (LPN) - Part-Time 11P-7A

Admissions, discharge, and transfers of residents; medication requisitions with pharmacy and physicians; medication administration; and delivering nursing care to patients requiring long-term or rehabilitative care. Collects patient data, makes observations, and reports pertinent information related to care. Identifies and reports abnormal changes in residents' condition to proper personnel per center policy. Collaborates with the resident's interdisciplinary team to provide the best outcomes. Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment. Follows infection control, emergencies, and other center policies. Maintains knowledge of necessary documentation requirements and equipment set-up, maintenance, and use. Maintains confidentiality and patient rights regarding all information. This role is part-time, 11P-7A. Florida AHCA Clearinghouse background screening is required.
