Licensed Practical Nurse (LPN) Overnight

Provide direct nursing care as Charge Nurse - LPN, conduct ongoing assessments of residents’ clinical status and contribute to individualized care planning with the interdisciplinary team. Deliver and oversee skilled nursing interventions (including wound care, IV therapy as permitted, and catheter care) and safely administer medications with accurate documentation. Serve as clinical leader on the unit by coordinating resident care, communicating with physicians and families, and supervising Certified Nursing Assistants and other staff. Ensure adherence to state and federal regulations, infection prevention practices, and Care Initiatives policies, while educating residents and families about care plans and health goals.
