Job Summary
Administer medications and immunizations as prescribed, monitor resident conditions and vital signs, and report changes to clinical staff. Provide wound care, support chronic condition management, and assist residents with activities of daily living including hygiene, mobility, and feeding. Document all care accurately in the electronic health record and educate residents on medications, wellness, and self-care practices. Collaborate with the multidisciplinary team to coordinate individualized care plans while following infection control protocols and responding to clinical emergencies. Full-time schedule with competitive compensation ranging from $70,720 to $79,040 per year.
Required Qualifications
- Active and unrestricted LPN license in any U.S. state
