Licensed Practical Nurse (LPN)

Part-Time LPN responsibilities include admissions, discharge and transfers of residents; medication requisitions with pharmacy and physicians; medication administration; adherence to infection control and center policies; collaboration with the interdisciplinary team to achieve optimal resident outcomes; identifying and reporting abnormal changes in residents’ condition; working under supervision per state nurse practice act and center policies; delivering care to residents requiring long-term or rehabilitative care; collecting patient data, making observations, and documenting pertinent information; implementing the patient plan of care and evaluating response; directing and supervising care provided by other nursing staff in specified situations; maintaining documentation requirements and knowledge of equipment and its use; ensuring confidentiality and resident rights. QUALIFICATIONS include active LPN license and CPR Certification; two years nursing experience preferred.
