Job Summary
Ensure individuals receive quality, individualized health care management through routine site visits, vital checks, assessments, medication counseling, and progress documentation. Act as a health resource by training staff on medical procedures and conduct audits to ensure compliance with state requirements and organizational policies regarding medical care and records. Participate in an on-call rotation for triage of medical concerns for one week per month. This full-time, Monday-through-Friday role includes occasional travel between Kokomo, Peru, and Lafayette areas, with a typical schedule of 8am to 5pm.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the State of Indiana
- Acceptable background check
- Driving record check
- Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Strong verbal/written communication skills
- Ability to work independently and efficiently
- Organizational and computer skills
- Must be available for occasional travel between the Kokomo/Peru/Lafayette areas
- Must be available for on-call rotation for triage of medical concerns for 1 week per month
- Full-time Monday-Friday schedule (40 hours)
- 8am-5pm shift

- 1-2 years experience working with individuals with disabilities or other long-term care
