Job Summary
Ensure individuals receive quality, individualized health care management through routine site visits, vital checks, medication administration counseling, and progress documentation. Act as a health resource for staff by training them in medical areas and conduct audits of medications to maintain compliance with state requirements and organizational policies regarding medical care and records. Perform semi-annual physical assessments and participate in an on-call rotation for triage of emergencies one week per month. This full-time, Monday-through-Friday role involves occasional travel between Kokomo, Peru, and Lafayette areas, with a typical schedule of 8am to 5pm.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the State of Indiana
- Acceptable background check
- Acceptable driving record check
- Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Strong verbal/written communication skills
- Ability to work independently and efficiently
- Organizational and computer skills
- Must be available for occasional travel between Kokomo/Peru/Lafayette areas
- Must be available for on-call rotation for triage of medical concerns for 1 week per month
- Full-time Monday-Friday schedule (40 hours)
- 8am-5pm shift
Desired Qualifications
- 1-2 years experience working with individuals with disabilities or other long-term care
