Job Summary
Licensed Practical Nurse (LPN) needed to support adults with developmental disabilities in New Castle, IN. Responsibilities include providing quality, individualized health care management; conducting routine site visits, taking vitals, performing assessments, counseling on medication administration, and documenting progress. Acts as a health/medical resource for staff, trains staff in medical areas, conducts audits of medications, and ensures compliance with state requirements and organizational policies. Schedule: full-time Monday–Friday (40 hours) with occasional travel and an on-call rotation for triage of medical concerns one week per month. Requires Indiana LPN license, strong communication and organizational skills, and ability to work independently within a collaborative team environment.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in Indiana
- 1-2 years experience working with individuals with disabilities or other long-term care is preferred
