Job Summary
Conduct routine site visits, check vitals, perform assessments, counsel on medication administration, and document progress for individuals with developmental disabilities. Act as a health and medical resource for staff by training them in the medical arena and conduct audits and reviews of medications. Ensure compliance with state requirements and organizational policies regarding medical care and records while managing semi-annual physical assessments. Work full-time Monday through Friday with occasional travel, including an on-call rotation for triage of emergencies for one week per month.
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the State of Indiana
- Acceptable background check
- Acceptable driving record check
- Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Strong verbal/written communication skills
- Ability to work independently and efficiently
- Organizational and computer skills
- Full-time Monday-Friday schedule
- Occasional travel required
- On-call rotation for triage of medical concerns for 1 week per month
Desired Qualifications
- 1-2 years experience working with individuals with disabilities or other long-term care
