Job Summary
Under the supervision of a Registered Nurse, the Licensed Practical Nurse (LPN) provides skilled nursing services according to the plan of care and state practice acts. Responsibilities include instructing patients/caregivers in care and disease management, documenting patient comprehension, observing patient responses, and recommending revisions to care plans. The position requires a current Licensed Practical Nurse license, graduation from a state-approved LPN program, and clinical experience.
Required Qualifications
- Unencumbered, current Licensed Practical Nurse license in the state where the HomeCare agency is located or compact state if applicable
- Graduate of a state approved LPN program
- Minimum one (1) year clinical experience as an LPN
- Individuals with less than one full year’s experience as an LPN will be considered with mentoring and oversight
Desired Qualifications
- Experience in home health care is desirable
