Job Summary
Provide hands-on, patient-centered care as a Per Diem LPN across multiple settings including hospitals, skilled nursing facilities, rehabilitation centers, nursing homes, and assisted living communities. Deliver care such as monitoring vital signs, administering medications, wound care, injections, blood draws, and catheter care. Admit, discharge, and transfer patients per facility policies; maintain clear, accurate documentation; collaborate with Registered Nurses and interdisciplinary teams; participate in team huddles; communicate effectively with patients, families, and colleagues; manage supplies and restock medications and equipment. Requires graduation from an accredited LPN program, active LPN license, CPR/BLS certification, ability to lift up to 80 pounds, COVID-19 vaccination as required, and prior per diem or travel nursing experience preferred.
Required Qualifications
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- Active, unrestricted LPN license in the state of practice
- Minimum 6 months of recent LPN experience (1–2 years preferred)
- Current CPR/BLS certification
- Ability to push, pull, support, and lift a minimum of 80 pounds
- Physical stamina to manage long hours of standing and walking
- Annual physical examination (where required)
- Negative TB test results
- Clean background check
- COVID-19 vaccination and boosters, as required by CMS or state regulations
