Job Summary
LPN provides and implements direct patient care in clients' homes on a Monday–Friday 8 AM–4 PM schedule, with one-on-one home health visits. Responsibilities include accurate documentation per the plan of care, educating family/caregivers, assisting RNs or physicians with procedures, preparing equipment, observing aseptic technique, and helping patients learn self-care. The role requires an active LPN license, CPR and PPD certifications, at least 1 year of LPN experience, a valid driver’s license, and reliable transportation.
