Job Summary
Provide daily nursing care, order medications, and update RNs and Nurse Practitioners while training and orienting AMAPs. Implement Individual Program Plans to promote participant independence and oversee their safety and well-being. Attend to personal needs, initiate peer support, and ensure compliance with government regulations and operating standards. Schedule appointments, participate in agency activities, and maintain equipment and supplies. Complete required documentation and travel to various locations.
Required Qualifications
- A.A.S. Degree or High School plus Specialized Trade Certificate
- Valid NYS Licensure as a Licensed Practical Nurse
- current NYS Registration Certificate
- Strong interpersonal and communication skills
- Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel)
- Travel is required
- valid NYS Driver's License
- Ability to attain and maintain SCIP-R and CPR certification
