Licensed Practical Nurse (LPN)

Admissions, discharge, and transfers of residents; medication requisitions with pharmacy and physicians; medication administration; following infection control, emergencies, and center policies; collaborating with the interdisciplinary team; identifying and reporting abnormal changes in residents' condition. Deliver nursing care to patients requiring long-term or rehabilitative care, collect data, make observations, and report pertinent information. Implement the patient/resident plan of care per state regulations, evaluate responses, and supervise care given by other nursing personnel in selected situations. Maintain documentation knowledge, equipment set-up and maintenance, confidentiality, and adherence to the Nurse Practice Act. Works under direct supervision in accordance with facility policies.
