Licensed Practical Nurse (LPN/LVN)

Licensed Practical Nurse (LPN) provides skilled nursing care under the direct supervision of a Registered Nurse. Assists the RN with the Plan of Care, performs selected procedures under physician orders, and participates in evaluating and refining client care plans. Documents clinical progress notes in EMR within 24 hours of visits; observes and reports changes in patient condition; supports patient education and self-care techniques; fosters teamwork and communication among staff, clients, and families; maintains TB testing and adheres to agency policies and dress code. The role requires nursing education from an accredited program and state licensure as an LPN, with home health experience preferred. The position emphasizes patient safety, emotional and physical comfort, and collaboration within an interdisciplinary team to deliver curative, rehabilitative, palliative, or preventative care.
