Job Summary
LPN/LVN (Part-Time) provides resident health assessments and develops care/service plans under the Director of Health and Wellness. Duties include documenting changes in condition per regulations, communicating resident needs to the health team, hands-on nursing tasks, reviewing and updating care plans to meet regulatory requirements, training care staff, transcribing physician orders, maintaining required certifications, CQI activities, MARs review, HIPAA and safety policy compliance, and contributing to a collaborative team focused on quality resident care. Must hold LPN/LVN licensure, have at least one year of nursing or caregiver experience (preferably in assisted living, home health, or memory care), and at least six months in a supervisory role. Proficient with Word/Excel and able to read and apply physician orders to MARs. Strong communication skills and ability to work with residents, families, physicians, and staff.
Required Qualifications
- High School Diploma or equivalent
- LPN/LVN licensure
- Minimum one year of nursing or caregiver experience (preferably in assisted living, home health, or memory care)
- Minimum six months supervisory experience
- Experience with Microsoft Word and Excel (preferred)
- Ability to read physician orders and match to MARs
- Understanding of medical terminology
- Excellent verbal and written communication skills
- Able to communicate verbally in person and over the phone
