Licensed Practical Nurse (LPN/LVN) - Full Time

Collaborate with departments to ensure employees receive appropriate care and programs. Formulate individualized programs based on health risk assessment results while targeting high-need areas with interventions. Assist with annual physicals, screenings, testing, and results reporting. Help schedule follow-up exams, return-to-work exams, and consultations. Coordinate client referrals to other providers and arrange follow-up evaluations with detailed documentation. Participate in formal classes, coordinate health fairs, and conduct training sessions for regulatory requirements. Collect and interpret data on program offerings to assure timely tabulations and evaluations. Provide one-on-one counseling and coaching with clients, interpret screening services, and consult on health awareness and care navigation. Build teams and partnerships with internal and external partners. Ensure a safe working environment by following regulatory policies. Work within electronic medical records to register patients and document visits. May require travel between clinics.
