Licensed Practical Nurse (LPN) | Long Term Care

Perform all licensed practical nursing functions and services as directed by the Registered Nurse, including completing resident assessments, making appropriate interventions, and prioritizing care using the nursing process. Administer medications safely and accurately in accordance with physician orders and facility policy, utilizing the Pyxis medication administration system. Participate in care planning processes, initiate updates to resident plans of care, and communicate changes in resident condition to physicians using the SBAR tool. Comply with professional documentation standards, investigate incidents and complaints, and facilitate conflict management while guiding staff in meeting resident needs. Work rotating shifts including days, evenings, nights, weekends, and holidays at Avera Mary House.
