Job Summary
Provides professional nursing care to residents within the LPN scope of practice in a Long Term Care setting. Performs licensed practical nursing functions as directed by the Registered Nurse, including assessments, planning, implementation, and evaluation of resident care. Administers medications safely and accurately per physician orders and facility policy, using Pyxis when applicable. Participates in the care planning process, initiates and updates the plan of care, and ensures care is implemented consistently and appropriately for residents. Maintains professional documentation and communicates changes in resident condition to the physician using SBAR. Guides and mentors staff, participates in physician rounds, and assists with orientation of new staff. Investigates incidents and resident/family concerns and reports promptly. Seeks educational opportunities to enhance practice and collaborates with the multidisciplinary team to move health forward for residents.
Required Qualifications
- Licensed Practical Nurse (LPN) - active license in the state of practice
- Ability to work rotating days/evenings/nights/weekends
- Minimum qualifications as outlined by facility policy and state nursing board
