Job Summary
Provides professional nursing care, assessment and evaluations for residents in long-term care as part of a multidisciplinary team. Administers medications and treatments as ordered, completes assessments, plans and updates care plans, and ensures safe medication administration using policy and Pyxis. Participates in physician rounds and care planning, documents thoroughly, uses SBAR for communication, and mentors/coordinates staff to deliver consistent resident care aligned with age-related needs. Seeks educational opportunities, supports orientation of new staff, and adheres to regulatory standards (CMS, Joint Commission, OSHA) and facility policies. Eligible for sign-on and relocation bonuses as offered.
Required Qualifications
- Licensed Practical Nurse (LPN) license in state of practice
- Active nursing license upon hire
- Less than 1 year experience in Long Term Care (preferred)
