Job Summary
Licensed Practical Nurse (LPN) needed for in-home Home Health care in Logansport, IN. Provide pediatric and adult patient care with G-tube feedings, trach and ventilator support, wound care, and medication administration. Conduct head-to-toe assessments, educate patients and caregivers on disease management and emergency procedures, collaborate with physicians and therapists, document care accurately, and uphold safety, infection control, and privacy standards. Flexible shift options (4–12 hour) with daytime, evening, and weekend opportunities; potential for long-term cases. Requires active Indiana (or compact) LPN license and CPR certification; 1–2 years of clinical nursing experience preferred.
Required Qualifications
- Active LPN license in Indiana or an active compact LPN license
- At least [1–2] years of clinical nursing experience (home health and pediatric experience strongly preferred)
- Experience with G-tube, trach, or vent care preferred (but training will be offered)
- Current CPR certification
- Strong critical thinking and independent decision-making abilities
- Excellent interpersonal and communication skills
