Job Summary
Provide patient care in accordance with accepted standards, the written plan of care, and the physician's treatment plan. Implement individualized care plans developed by the Nurse Manager and interdisciplinary team. Perform medication administration and prescribed treatments while monitoring residents' conditions and reporting changes promptly. Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards. Support a safe, respectful, and therapeutic environment for residents and their families. Full-time and part-time opportunities available with shifts including 7am-3pm, 3pm-11pm, and 11pm-7am. Current, unencumbered active LPN license in Rhode Island and valid BLS Certification required. Join a collaborative team at Lincolnwood Rehabilitation and Healthcare Center, a 200-bed Sub-Acute and Long-Term Care facility designed with home-like common spaces.
Required Qualifications
- Current, unencumbered active LPN license in the state of Rhode Island
- Valid BLS Certification (CPR)
- Ability to care for and manage multiple patients in a fast-paced healthcare environment
- Strong clinical judgment
- Communication skills
- Attention to detail
