Job Summary

Provide healthcare oversight and medication coordination for individuals with intellectual and developmental disabilities in community-based programs and home visits. Conduct supervisory home visits every 60 days, document care plans and physicals, and ensure compliance with Medicaid waiver services, CARF standards, and agency policies. Collaborate with staff, families, and medical professionals to facilitate health management, attend Individual Service Plan meetings, and manage on-call rotation for overnight and weekend coverage. Maintain accurate documentation to support quality care and regulatory adherence. This part-time role reports to the Registered Nurse in the Developmental Disabilities Division, requiring a Georgia LPN license and 5 years of nursing experience.

Required Qualifications

  • Active, in-good-standing Georgia LPN license
  • Active, in-good-standing Georgia driver's license
  • 5 years of experience as a Licensed Practical Nurse
  • Computer proficiency
  • Ability to maintain required 16 h annual training (DBHDD)
  • Must be able to lift 50 lb or less
  • Must be able to work a flexible schedule
  • Must successfully complete a pre-employment background screening
  • Must successfully complete a drug screening
  • Must pass tuberculosis (TB) testing
  • Must complete fingerprinting
