Job Summary
Provide in-home direct patient care per the provider's prescribed plan of care, monitor progress/recovery, administer medications (including insulin and IV fluids), manage wound care and dressing changes, assess patient conditions and report changes, and collaborate with healthcare teams to ensure optimal patient outcomes.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as a LPN/LVN in the state of assignment
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
