Job Summary
Provide in-home nursing care for patients in the Rosemount, MN area, performing ongoing interdisciplinary assessments, disease and medication management, and skilled wound care. Deliver catheter management, ostomy education, infection prevention, and home-safety guidance; document timely clinical notes; participate in planning and evaluating the care plan in partnership with patients and caregivers. Travel within the designated geographic area to patient homes; collaborate with an interdisciplinary team; flexible scheduling with weekend/holiday rotations. Required Qualifications
- Current MN LPN license in good standing
- CPR Certification
