Job Summary
Licensed Practical Nurse (LPN) role providing in-home direct patient care per prescribed plan of care with one-on-one patient focus. Responsibilities include assessing and documenting patient conditions, administering medications (including insulin and IV/fluids), wound care and dressing changes, monitoring progress, reporting changes, and collaborating with providers and allied health professionals to ensure positive patient outcomes. Requires active LPN license in the state, applicable CPR/BLS/First Aid certifications, and transportation to care sites in Greensburg and surrounding areas; flexible FT/PT/PRN scheduling is offered.
Required Qualifications
- Graduate of a practical nursing program for LPN/LVN
- Current license as LPN/LVN in the state of employment
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
