Licensed Practical Nurse (LPN)

Job Summary
As an LPN, provide in-home direct patient care under the prescribed plan of care, including monitoring progress, administering medications (insulin/IV/fluids), and wound care. Assess and document patient conditions (vital signs, labs, wound healing) and report changes as needed. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. Maintain thorough documentation and ensure patient comfort and dignity in the home setting. Positions available in Greensburg and surrounding areas with flexible scheduling (FT, PT, PRN).
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the state where position is located
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred
