Job Summary
In-home Licensed Practical Nurse (LPN) providing direct patient care per the provider's plan of care, including vitals, medication administration (including insulin/IV fluids), wound care, and documentation. Responsibilities include assessing patient conditions, reporting changes, assisting with hygiene and dressings, and collaborating with providers, aides, and other health professionals to ensure positive patient outcomes. Requires graduation from a practical nursing program and an active LPN/LVN license in the applicable state, current CPR/AED/BLS/First Aid, and reliable transportation to care sites. Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the state of position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
